Privacy
Contra Costa Health Services (CCHS) is committed to protecting the privacy of visitors to its website.
CCHS only collects personal, identifiable information, including specific email addresses, from site visitors, when site visitors voluntarily and explicity provide it. This information is collected when site visitors use CCHS email and respond to online surveys. Surveys conducted on CCHS websites will not require visitors' names or medical records information.
For visitors who wish to receive a response to an inquiry, information such as name and address will be required so CCHS staff can respond. That personal information will not be used for purposes other than to respond to the inquiry without permission.
CCHS collects data on website activity for the purpose of stastical analysis of site performance. Information such as a visitor's IP address, Internet service provider, date and time of visit, the link that led to the CCHS site may be collected. Summaries of the results are available to CCHS staff and public, and contain no personally identifying information. CCHS may be required by law to disclose information in response to various Freedom of Information and other requests from private individuals or governmental bodies. CCHS also shares server activity information with law enforcement officials when indications of criminal activity or intent to commit such activity are present.
CCHS staff may use information submitted by website users through email in carrying out their official duties and responsibilities and to respond to inquiries. There may be instances where information from visitor email may be sent to the person or company about whom the user is complaining, or to other governmental agencies involved in the administration and enforcement of the matter. Email sent to CCHS should not be considered secure or confidential. Email to CCHS, like all non-encrypted Internet email communications, may be accessed and viewed without visitors' knowledge or permission while in transit to CCHS. Visitors who have confidential information to communicate may want to contact CCHS by telephone at the numbers provided for various locations on CCHS sites or by surface mail. CCHS encourages visitors not to disclose any personal identification information in electronic format other than basic contact information. CCHS discourages the submission of personal identifier information including social security, or case management numbers.
